Hi Soumya,
First you have to "Activate" the Questionnaire after completion of Questionnaire creation . And once again you have to "Active"(check the box) the Questionnaire while assigning to "Lead" Transaction type.These are the necessary things to display Lead Questionnaire in Web UI.
